About Me
Hi, I'm Dr. Thomas Sargent. I take time to explain what I recommend and why, because every medication should truly earn its place in your treatment. My goal is for you to feel confident in your plan and like someone is paying close attention to how it's going.
I keep my practice intentionally small.

Most people who reach out have been managing on their own for a while. Focus problems that are starting to cost you at work, or worry that hums under everything. Mornings that start heavy and stay that way. Sleep that doesn't restore you. A medication that helped at first, then stopped helping. If any of this sounds familiar, you're in the right place. You don't need the right words before you book. Finding them together is what the first appointment is for.
I have a master's in pharmacology alongside my MD, so I can explain what a medication does, why it fits you specifically, and how it interacts with everything else you take. That training works in both directions: finding the right medication, and carefully simplifying a list that has grown over the years.

Payment is due at the time of service for all visits.
The care is the same however you pay. Private pay just changes a few practical details.
Out-of-Network? You can still get reimbursed.
Many PPO plans reimburse 50-80% of treatment costs. I provide a monthly superbill with all CPT and ICD-10 codes your insurance needs to process your claim.
The Process:
Example: If your plan reimburses 70% after your out-of-network deductible, a $500 initial evaluation would cost you $150 out of pocket.
Tip: Call the number on your insurance card and ask: "Do I have out-of-network benefits for outpatient mental health telehealth services?"

I work with adults 18 to 65 in New York and Texas who want ongoing outpatient care: a thorough evaluation, careful medication management, and steady follow-up. Because this is a solo telehealth practice, I cannot provide crisis services, and some situations are better served by a higher level of care than outpatient visits can offer. If you're not sure, book a free 15-minute call and we'll talk it through together either way.
Your first appointment is a 60-minute video evaluation via a secure, HIPAA-compliant platform. We'll talk about what brought you in, your history, and what you've already tried. By the end, you'll have a clear picture of what I think is going on and a plan for next steps, whether that includes medication or not.
After that, follow-up visits are 30 minutes. Early in treatment, we typically meet every two to four weeks so I can monitor how things are going and make adjustments. Once you're feeling stable, we can space visits out further.
A phone, tablet, or computer with a camera, a reasonably private place to talk, and you must be physically in New York or Texas at the time of the visit. Your telehealth link arrives by email before each appointment.
That's very common. I'll review everything you're currently taking during your initial evaluation and we'll talk through what's working and what isn't. If your current regimen makes sense, I'm happy to continue it. If I have concerns, I'll explain why and we'll work on a plan together. Either way, I'll make sure there's no unsafe gap in your prescriptions during the transition.
Yes, when clinically appropriate. I take a careful, safety-first approach to prescribing. ADHD diagnoses require a thorough evaluation, and I generally do not prescribe long-term benzodiazepines as a first-line treatment. I review the Prescription Monitoring Program for all controlled substance prescriptions.
Both, in the same visit. Follow-ups are a full 30 minutes, which leaves real room to talk through what came up since we last met and work on practical skills, then tie it back to how your treatment is going.
We define that together at the start, then we measure it as we go. I use the same symptom rating scales used in clinical research, alongside the things that matter to you personally. Maybe that's sleeping through the night, or getting through a workday without white-knuckling it, or just feeling like yourself again around the people you love. If something isn't working, we change it.
Yes. You're welcome to join as a private pay patient whether or not you have insurance. The initial evaluation is $500 and follow-ups are $250, and HSA and FSA cards are accepted.
Yes, through the secure patient portal. I'm the only one who reads those messages. Medication questions, side effects, and scheduling are usually handled there within a business day or two. The portal isn't monitored around the clock, though, so for anything urgent or unsafe, use the emergency resources below.
If you need to cancel or reschedule, please give at least 48 hours' notice. Late cancellations and no-shows are charged $150.
I provide enough medication to last until our next appointment. Need a refill between visits? Contact me via the patient portal at least 3 business days before you run out. Controlled substances cannot be refilled early or after hours.
My practice is not an emergency service. If you are experiencing a crisis, call 911, go to your nearest ER, or call the Suicide & Crisis Lifeline at 988.
